This lady underwent a combination of open ultrasonic septorhinoplasty and alar base reduction to feminise the nasal appearance and optimise nasal function. A combination of techniques was used to correct the underlying septal deviation and feminise the side profile. The deviation was corrected using a wafer thin buttress of bone taken from the septum – a powerful technique to provide a robust correction without widening the nose from the front. The nostril width was reduced through separate incisions at the base of the nose (alar base reduction). At 3 months most-op, the scars are barely conspicuous.
