A 19 year-old lady underwent open revision rhinoplasty to reduce a dorsal bump and refine the tip shape following an unsuccessful septorhinoplasty for nasal trauma 2 years previously. A feminine side profile with supra-tip break was achieved through a component reduction of the dorsum and columella strut graft with tongue-in groove sutures. Creation of dorsal aesthetic lines on the front axis was achieved through percutaneous osteotomies, auto-spreader grafts and tip suture techniques. Result at 3 years.
